Create TV Scanlines

Create a new transparent canvas with dimensions of 10x4. Zoom in all the way (1600%). Get out the Single Row marquee tool and click up the top of your canvas. Fill with black. With your selection still active, hit the down button twice and fill with black. Go to Edit > Define Pattern and type in TV Scanlines. Done!

To apply the scanlines, open up an image and go to Edit > Fill. Select pattern from the dropdown menu and select the TV Scanlines pattern from the Custom Pattern section. Set the Blend Mode to Overlay and the opacity to 100%. Hit OK.
