on menu book 20 I've fitted a custom wing but game not progressing?

Did you fit the last wing on the list, with the adjustable height and endplates, or just one of the cosmetic, unadjustable ones?

fitted to 3 cars now but nothing i have fitted one before maybe getting into the first car a fitted one to will work?

This is quite a difficult sentence to parse. However I think you're trying to say that you've fitted wings to cars before the Menu Book and they're not counting.

For reference, they don't: you have to start the Menu Book, then fit a wing to a car to complete it. Meeting the requirements before you start the book doesn't count.

Well, in my case it counted a wing previously installed.
I had installed a wing previously in the Subaru BRZ STi. When I accepted the menu, I thought I should install a wing at another one, exited café to go to garage select a car, but as soon as I exited café, saw te message that should go to café to conclude the menu. Never had to install another wing.
 
I completed every "upgrade the car" menu with a car that had been upgraded before the the menu was started. Accept the mission, go to guagre and switch to a car that meets the criteria, then the "lets go back to the cafe" should pop up from the top menu. At this point head back to Lucas and get your reward.

I guess that's changed since v1.02 pre-launch when I did it (although it would appear that subsequent updates have borked a lot of Menu Books too), as it very definitely didn't work! I lightly shouted at the game for that.

Well, can't be sure, but I should have the GT-R R32 from the races of Menu 19, which had also a upgraded rear wing but not a custom one. But there was no need to change car, that I'm sure.
