Music Sounds is the 2016 release from Mimi Betinis, long-time session guitarist and lead guitar of Pezband, a '70s power pop act from Illinois. The jacket was printed exactly as it appears above, with the appearance of poor condition being a part of its design.
Let's look at the oposite end of the spectrum. I was going for a Steely Dan album when I accidentally slid this out instead, and while I enjoy the music (it's in my record collection), it has to be one of the worst covers of all time.

The outside cover of Close to the Edge is one of the more minimalist Yes album covers (It's just the Yes logo and the album title on a black to green gradient) but the inside cover not so much.
The common thread between these albums--apart from their release during a five year span in the latter half of the '70s--is their covers were composed by comedian Phil Hartman, best known for his work on Saturday Night Live, NewsRadio and as the voices of Lionel Hutz and Troy McLure on The Simpsons.

My local Andy’s Records had a plastic mould of this face coming out of the wall when they were promoting this album, I begged and begged but they wouldn’t sell it to me. Always loved the inside gatefold artwork too.
In fact, it looks to be a '67 or later when the chrome trim was changed to extend straight back on the bodyside instead of curving up on the door to the top of the rear fender.
