I've noticed that as well, but they have always shown up as a friend within a day or so.

Found the answer: Apparently the online GT Sport profile isn't connected directly to the PSN friend list. When you fire up the game on your console, it finds the new PSN friend connection and then notifies the GTS servers. Then, you'll have the new friend in the game.
