This snail got infected by a bird-poo-borne parasite.
The eggs hatch inside the body of the snail, and the larvae grow INSIDE the snails eye-stalks.
And they live.
...only to be ripped off by a bird which sees them pulsating like maggots.
This vicious cycle is both perpetuated by, and serves to sustain the life of both the worm and the bird: The bird gets its' food, and then passes on the eggs in its' droppings. The snail eats the droppings, ingesting the eggs. The eggs hatch inside the snail, growing up inside the translucent stalks of the snail's eyes.
The snail ordinarily survives having his eyes ripped off, and will grow them back. . . with another parasite to fill them.

Yeah, I thought that scene itself was shot in Japan, with the some of the others set up in LA.

I believe the scenes where they are drifting through the city were filmed in LA, and most of the walking/talking scenes were in Tokyo (as well as some of the parking garage parts).
