Well when they appear, I've seen cars go off and no flag ever came out. Nearly got hit by one of the said no yellow crashed cars flying back on at a sharp angle too.
There are many issue with the yellows and penalties right now, even if the video in the op isn't one, it's possible removing the system is an easier, therefore more likely solution than fixing it.

Ok, but if people are spinning out ahead of me, I'm grateful for the warning ahead of time so when they are trying to rejoin the track I'm at least aware there will be slow moving cars around.
So, because sometimes they don't work they should be abandoned completely? They need working, but removing them would be a step back imo

It is much more likely to happen to you if you are not at the front as the main cause of unfair penalties (not the only one) is when you pass the car that caused the yellow when it has just been auto reset to track, pass before that and it's usually fine unless multiple cars have gone off.
I like the yellow flags for the warning they provide, penalties make them more exciting and possibly race changing sometimes getting a benefit and sometimes losing out.

But that happens in real life, eg use up tires getting a 20 second lead and then the safety car comes out.

No but then I worry that PD fixing this will actually make it worse than it currently is. I'd argue this yellow system shouldn't have been added until it was working better than it currently is in the first place too.
So, you're worried that, in trying to make it better PD might make it worse... which justifies removing it?
