The settings are always geared toward avoiding this, as far as I know, but there are some instances where the game decides to reset the car. This is on PD not allowing cars to take actual designated run offs for missed or blown corners. This will not result in the drivers being held accountable if appropriate attempts are made not to be reset.
This would at times fall under the "out of control" rule, meaning when all control is lost sometimes the car does something the driver does not intend, but lets say you try to full-power and cut a Monza chicane, based on inputs we can see if you made some attempt at avoiding or just tried to blow it, and if the latter's the case that may fall into IR territory.
Under normal circumstances, if you brake, slow down and gradually re-enter the track, there should be no reset, and if that happens its easy to tell if it falls under PD or driver error.
