Sometimes the reason is because you're trying to focus on something too close to the camera.(Sorry for the terrible quality, both my phone and my DSLR can't focus on it properly.)

Isn't it Tarmac Works? The OM DC2 came out a long time ago. And that thing was out of this world expensive.
