Well, I actually have 4 names, but since the last two are surnames (3rd and last name are mother and father surnames, respectively, I used my 2nd (which will probably be the common known middle name) and the first name of my street. I knew Miguel Barbosa sounded familiar.

I can't do the dog/street version, since I no longer have a dog , but If I still had it, it would become something like Scot Barbosa or Scot du Bocage. The last one sounds kind of French.
