It's not that hard, you just flick the throttle with the outside of your foot, you don't actually have to use your heel- but in GT5 it isn't really that necessary.

It's not too hard in GT5, but in iRacing where it is basically necessary to keep the car under control, it's very hard.

Yep, but that's because GT5 doesn't simulate the lurch when you (down)shift.
Having said that, the Lotus Elise in stock form is the only car I have seen any need to Heel & Toe in.
