I think you might be mixing up Naoki Hattori (who racked up two DNPQs for Coloni late in 1991) and Masami Kuwashima, who withdrew from the 1976 Japanese GP after just one practice session.

It's Apicella. He started one race at Monza and made it only to the first chicane.

Ding ding, we have a winner... @DK got there first.

Apicella's F3000 performances earned him a drive in the Jordan at Monza. 800 metres after the start he was out of the race and was replaced for the next race by another driver.

Off the top of my head, Rubens Barrichello in 2001 could be one. The other? Probably Mark Webber in 2011.
 
Which non-sovereign region bidded for a Formula 1 Grand Prix in 1986, but had it rejected after a test event the previous year found it unsuitable for even the lower formula cars.

Yes, Curaçao came close to hosting an F1 race in 1986, and used the 1985 F3000 race as a test. The circuit wasn't right for even them, so F1 never appeared there.

General Trivium:

In the 1989 British Touring Car Championship, John Cleland was partnered throughout the season by Louise Aitken-Walker. While Cleland had a proper, race-prepared Astra provided by Dave Cook Racing, and won the 1989 title, the team only actually had one race-spec BTCC car. Aitken-Walker drove her rally Astra set up for tarmac conditions (And finished 2nd in class and 5th overall). The other Vauxhall Astra of Jeremy Rossiter was a National Saloons car.

General trivia.

The 12 Hours of Sebring has hosted both a round in the World Endurance Championship and the North American Endurance Championship (albeit the NAEC is a Championship in a Championship).
 
A one season lasting rally series called Formula Rally Championship was held in UK in 2001, why was the series held for one season only and who won the championship.

Winner: Mats Andersen (Car was a Satrian Kit Car).

Reason for 1 Season: It was the replacement for the cancelled 2001 British Rally Championship which eventually came back in 2002.
