No. Portsmouth would have been one but for one technicality - they didn't play in Europe the following season.

They were relegated from 20th place in 2009/10 and having reached the FA Cup final - which they lost to Chelsea who qualified for the Champions' League through winning the Premier League and thus not been able to take the UEFA Europa League place for the Cup win - they would have been eligible for the Europa League. But they had been placed in administration and lost 9pt - UEFA barred the club from competing in European competition and gave the place to Liverpool (the next highest league team) instead.
Oh, I forgot about that. I know they had technically qualified but forgot about the administration thing.

Yep! Relegated from 18th place in 2001/02, but qualified for the 2002/03 UEFA Cup under the FIFA Fair Play Award.

Could Millwall count, or were they not in the top division in 2003/4 when they were runners up in the FA Cup, if that's a path to European football?

No. They weren't relegated from the top flight the same season as they qualified.
 
Yep. Crystal Palace finished 20th 1997/98 and were relegated but inexplicably qualified for the UEFA Intertoto Cup in 1998. They were knocked out in the 3rd round.

Which two players have made the most apperances at the european champions??
 
First thought would be an Italian player as they seem to play into their late 30s to 40s.

Surely one of them will be a goalkeeper though as they normally retire later, I know Iker Casillas has over 120 caps (may be close to 130 caps now) so I will say him for sure. Still fairly young for a keeper as well, not to mention he is regarded as one of the best keepers in the world. He will surely earn a fair few more caps as well :)

Edit: Noticed question was just for Euros (not including world cup)

I'm going to change country and say Lothar Matthaus. He's been around for a while and Germany always do well in the Euros.
 
I agreed with the theory on goalies, and once you've dropped Germany, Italy and France, there's only one country really left it can be.

I'll waive my equal-going-rights.
